This program is a result of the expressed need from families for loved ones requiring care.
For over a decade, the Community Services Department and now the Recreation and Community Services Department has provided scholarships for physically and mentally handicapped and frail, low-income adults through a number of local non-profit adult day care providers. The Grant Compliance Unit within the department provides the administration of the Adult Day Care program. The City of Jacksonville contracts with local providers for the delivery of adult day care services. The project manger monitors each provider's performance on a bi-annual basis, in accordance with Community Development Block Grant (CDBG) guidelines.
The program's goal is to provide adult day care services to 45 unduplicated physically and mentally handicapped and frail, low-income adults by Sept. 30, 2010. Achieving this goal will reduce the financial burden to low/moderate income clientele and their families. This goal directly relates to our goal of delaying the institutionalization of 85 percent of participants by Sept. 30, 2010. The provider agencies accomplish these goals through a comprehensive treatment plan that consists of nursing care, case management, meals, transportation, and recreational activities. For fiscal year 2009, the program is currently providing care to 42 unduplicated physically and mentally handicapped and frail, low-income adults.
