Items for New Clients to Bring Into The Office when seeing a Veterans Service Officer or Social Services Specialist. (If the below items are unavailable, we will provide you assistance in obtaining them)

A.  If possible, you should bring the following items (as applicable to you) with
     you when you see one of our Veteran Service Officers:

     1.  The Veteran's Military Discharge (DD-214)
     2.  The Veteran's Social Security number (and the social security numbers for
          immediate family members)
     3.  Marriage certificate
     4.  Birth certificate of child or children
     5.  Previous divorce decrees of veteran
     6.  Previous divorce decrees of current spouse
     7.  Death certificate
     8.  Original military medical records, if in your possession, or a copy
     9.  Receipts for funeral and cemetery expenses
   10.  Receipts for grave marker and/or engraving
   11.  Letter from nursing home indicating:
               a.  Date of admission
               b.  Level of care of the patient
               c.  Normal cost of care per day
   12.  Verification of income from the following:
               a.  Social Security Administration
               b.  Civil Service Annuity
               c.  Railroad Retirement Board
               d.  Private company pension
               e.  Other sources

B.  If possible, you should bring the following items (as applicable to you) with
     you when you see one of our Social Services Specialists:

      1.  The Veteran's Military Discharge (DD-214)
      2.  Identification with photo
      3.  Your resume, if you are looking for employment
