The Jacksonville Sheriff's Office administers the application and issuance of alarm registration decals as required by the City of Jacksonille Municipal Code. Click here for Municipal Ordinance section 168.102 which explains the requirements.
The required fee is $10.
Failure to properly register, operate or maintain any such alarm is subject to civil penalty. Click here for Municipal Ordinance sections 168.104 and 168.106 which explains the requirements and penalties.
Application for registration of a residential alarm, business alarm, or commercial installer/monitoring operation shall be submitted to the Jacksonville Sheriff's Office:

Alarm Contractors
In addition to any requirements established within the City of Jacksonville Municipal Code (Section 168), all alarm contractors must also comply with Section 342.
