Where is the business located?

What is the telephone number of the business?

How did you find out about this business?  Advertisement?  Yellow Pages?  Other?

What is the business (hours of operation)?

When did you first contact the business?

When did you first go to the business?  Date?  Time?

For what reason did you go to the business?

With whom did you come into contact at the business?  Name?  Address?

Description:   Age?  Date of birth?  Race?  Height?  Weight?  Hair?  Eyes?  Scars/marks?  Driver's license number?

What position did this individual hold with the business?

What statements were made by this individual?

Were there any other individuals with whom you communicated?   Name?  Description?

Were there any witnesses to the statements made by the above-referenced individuals?

What product or service did you purchase?

Did you execute a contract?  Do you have a copy?

Did you have any oral agreements?

Did you pay for the products or service?  How?  (Cash, check, money order, credit card)

Do you have a receipt, cancelled check, copy of the money order, or credit card statements?

Did the business provide the product or service?  What was provided?  Problems?

When you determined that there was a problem with the product or service, or that the product or service was not provided, who did you contact?

How did you contact the business?  When?

How did this individual identify himself/herself?

What did you tell this individual?

What was his/her response?  Was a refund offered?  If so, on what terms?  Conditions?

Did you speak with any other individual associated with the business?  Name?  When?

Was a police report filed with a law enforcement agency? CCR number?  Name of investigating officers?  Status of investigation?

Is there any additional documentation that was provided by the business?  Brochures?  Fliers?  Manuals?  Warranty?  Or any other documentation that might be helpful towards reviewing your complaint?
