You will be asked to complete an online survey before attending the training and follow-up surveys at 3 and 6 months after the training. Your answers will remain confidential. Everyone who completes the evaluation, including attending training and completing all three surveys, will be entered into a $100 gift card raffle. Participation is voluntary.
