2010 Annual Report

The Adult Day Care program is a result of an expressed need from families for loved ones living in Jacksonville, Florida requiring care.

For over a decade, the Recreation and Community Services Department has provided scholarships for physically and mentally handicapped and frail, low-income adults through local non-profit adult day care providers. The Grant Compliance Unit within the department provides the administration of the Adult Day Care program. The City of Jacksonville contracts with local providers for the delivery of adult day care services and the Grant Compliance Unit staff is responsible for monitoring each provider's performance on a bi-annual basis, in accordance with Community Development Block Grant (CDBG) guidelines.

The overall program goal is to provide adult day care services to 45 unduplicated physically and mentally handicapped and frail, low-income adults. Achieving this goal will reduce the financial burden to low/moderate income clientele and their families and directly correlates with our goal of delaying the institutionalization of 85 percent of the adult day care participants. The provider agencies accomplish these goals through a comprehensive individual treatment plan that consists of nursing care, case management, meals, transportation, and recreational activities.
