We are pleased to provide electronic versions of several of our applications for your convenience.
Applications (and supporting documentation, when applicable) should be submitted before July 31 to ensure a decision is rendered before the tax bills are issued. Tax bills will be amended if an application is approved after the bill is issued.
Before using these forms, please be aware of the following:
- Electronic applications function best when used in Adobe Reader 9 or higher, or Internet Explorer. "Submit by e-mail" does not work in Chrome PDF Viewer.
- We strongly recommend that you print a copy of your application for your records.
- Don't forget to include the required supporting documentation with your submittal. Incomplete applications will be rejected on receipt!
We have made every effort to make the electronic applications as user-friendly as possible. However, if you are unable to submit using the "E-mail" button on the form, please attempt to save the completed form and attach it to an email to firstname.lastname@example.org.
