The purpose of the Complaint Form is to start a review of a case by the Ethics Commission if it is in their jurisdiction. Click on the link below to download the form, then print it to fill in the information. When the information is completely filled in, ensure all back materials are attached, and sign it in the presence of, and have it co-signed by, a person authorized to administer oaths or notary public.
Mark the entire package “Confidential.” The complaint form may be scanned and notarized package may be emailed to email@example.com. Or deliver the printed materials in person or by mail to the Ethics Officer, Suite 225, City Hall, 117 West Duval Street, Jacksonville, Florida, 32202.
