Autopsy Reports

To submit a request for an autopsy report, please click here.
 
Autopsy reports may also be requested in writing by mailing a self-addressed, stamped business envelope with postage for two ounces to:

For a copy of a death certificate, please contact the funeral home or the Office of Vital Statistics in the county where the death occurred or at the state office.
